Admissions

Finding the right fit, one step at a time.

We know choosing a residential home is a big decision. Our admissions process is designed to be transparent, unhurried, and centered on whether we're truly the right home for your loved one.

1

Initial inquiry

Reach out by phone, email, or our contact form. We'll ask a few basic questions about your loved one's needs and current support to see if it sounds like a potential fit.

2

Tour & conversation

Visit Centered Care Home, meet staff and (where appropriate) current residents, and ask anything — staffing, routines, medical support, house rules.

3

Assessment & documentation

We'll request relevant records — diagnostic history, current support plan, medical information, and funding/waiver documentation — to confirm we can meet care needs.

4

Placement decision

Our team reviews the assessment together with you and your care coordinator, and confirms availability, funding approval, and a target move-in date.

5

Move-in & transition support

We build a personalized transition plan — including a gradual visit schedule where helpful — so the first days feel familiar, not overwhelming.

Eligibility

Who we support

  • Individuals with an intellectual or developmental disability diagnosis
  • Approved funding source or Medicaid waiver, where applicable
  • Support needs that match our current staffing and licensing level
  • Willingness to participate in a short trial/transition visit before move-in

Not sure if this describes your situation? Reach out anyway — we're happy to talk it through.
