Reach out by phone, email, or our contact form. We'll ask a few basic questions about your loved one's needs and current support to see if it sounds like a potential fit.
Visit Centered Care Home, meet staff and (where appropriate) current residents, and ask anything — staffing, routines, medical support, house rules.
We'll request relevant records — diagnostic history, current support plan, medical information, and funding/waiver documentation — to confirm we can meet care needs.
Our team reviews the assessment together with you and your care coordinator, and confirms availability, funding approval, and a target move-in date.
We build a personalized transition plan — including a gradual visit schedule where helpful — so the first days feel familiar, not overwhelming.

FAQ
Typically 1–3 weeks from initial inquiry to move-in, depending on funding approval and documentation.
We work with Texas HCS (Home and Community-based Services) Medicaid waiver funding, other state funding sources, and private pay.
We maintain an open-door visitation policy. We simply ask that visits outside normal hours be scheduled in advance with staff.
Staff are trained in first aid/CPR and follow individualized emergency protocols. Families are contacted immediately, in line with each resident's care plan.
